A study to analyze mortality rate and medical expenses by cohort study of patient with severe diseases (2004~2016):Focusing on analysis of the effect from the policy to enhance the coverage of National Health Insurance

Abstract
Objectives:We identified the incidence and mortality rate of 3 major diseases such as cancer, cerebrovascular disease and heart disease and reviewed the effectiveness of National Health Insurance benefit coverage strengthening policy based on the analysis of medical fee before and after applying 5% co-payment rate.

Methods:We used customized DB data of National Health Insurance and selected patients who were treated for malignant cancer (C00-C97, D45-D47) or cerebrovascular disease (I60-I63) or heart disease (I21-I23), respectively.

Results:The incidence of 3 major diseases was 0.27% for malignant cancer, 0.21% for cerebrovascular disease, and 0.03% for acute heart disease. As of 2016, after 12 years, mortality ratewas 51.2% for malignant cancer, 49.5% for cerebrovascular disease and 48.2% for acute heart disease. The result of analysis of medical expenses shows that the coverage ratio of National Health Insurance was very low as 90% for malignant cancer, 89% for acute heart disease and 80% for cerebrovascular disease.

Conclusion: The effects of National Health Insurance¡¯s policy for strengthening its coverage have been very small and, therefore, it is needed more aggressive and differentiated policy plan.
